Housebreaking my pup has been a nightmare. What would you suggest we do?

Full Question:
I have a 3.8 pound Morkie. She will be 7 months on the 23rd. She is a good dog. Lot's of personality but...the housebreaking has been a nightmare. I started crate training her and she was peeing in the kennel at night even though I walked her during the night. Then she stopped doing this and recently started again. She was kept in the crate during the day and walked once sometimes twice a day until I got home at 3:30. But she has always had accidents in the house no matter how many times I walked her. I would find pee spots on the carpet. I am honestly fed up. So I started during the day putting down a pee pad because I started a new job and I am gone all day and I can't come home on lunch as I have an hour commute. She will pee on the pee pad during the day when I am gone.... but she will pee wherever she feels the need, including my couch, when I am home with her even though she is walked every single hour! I feel as though I have confused her as she can't tell me or doesn't know how to tell me when she has to go so she just goes. Plus the pee pad I know has given her the idea that peeing or pooping in the house is ok. My husband wants me to get rid of her as she had an accident on the couch today and he is done with it all he says. I really need some guidance.

I’d get her to the vet for an exam, she may have a urinary tract infection.

If she checks out ok, then it’s a training issue.

I’d use a crate and an exercise pen with her just like I do with any other dog, regardless of size or breed. Don’t let her practice behavior you don’t want.
