I think the best person to ask would be a pharmacist. They could probably suggest a ratio, and they may sell the sulphur also. They'll probably suggest petrolatum instead of lard (updated medicine and all that). A pharmacist may even make the ointment up or may already have premixed.
Also, Bag Balm (sold in horse/cow feed stores) is already premixed and has sulphur in it. Don't know if the sulphur content is enough for the dog's condition. You might want to look up Bag Balm on the net. "Ingredients: 8-Hydroxyquinoline Sulfate 0.3% in a Petrolatum, Lanolin Base"
Here's a link I found, titled American Journal of Pharmacy, year 1883, where the following is taken from: "UNGUENTUM SULPHURIS.—Sulphur Ointment.—Sublimed sulphur, thirty parts; petrolatum, seventy parts. Rub the sulphur with the petrolatum, gradually added until they are thoroughly mixed. Probably no better than the officinal." I'd ask a pharmacist, explaining the condition of the dog.
The old farmers remedy I recall was for mange and not for an autoimmune disorder was sulfur (flowers of sulfur obtained from a pharmacy) and burnt motor oil.
Had a dog treated this way when I was a kid MANY years ago (1960s) and it was messy and not particularly effective.
Obviously burnt motor oil is now known to be carcinogenic and that dog actually did later develop skin cancer. In todays world, I think she would have been diagnosed with a flea allergy (not on snout but base of tail) or some other allergy/autoimmune issue.
I would also discuss any concerns with yout vet about the dog ingesting the sulfur (I know they used to dose human kids with sulfur for worms, but they also gave them turpentine for a cough.........)
