Stephanie,
Simple solution is to make sure you clip the lead on the collar before even opening the door. Then, keep the dog at your side (keeping zero slack in the lead) as you open the door. If they start to bolt, just stand solid and let them be confused. As soon as they look at you, bring them back to your side, and take one SMALL step forward after a heartbeat or two.
You are teaching them that they go out when YOU are ready to go out. Now that they are back by your side, you kinda lean back with the lead, keeping them in the same spot, as YOU walk over the threshold first. If they bolt, same thing: bring them up short, and back to your side. Make sure that they don't cross before you. If they do, bring them back inside, close the door and unclip the lead; you'll try again in a minute or so.
Does this mean it sometimes takes 20-30 minutes to get your dog out? Sure does! But if they really need to go, they will pick up on it quickly. The point is that YOU, the pack leader, goes out first.
Just think of it in almost the same way as taking a barn-sour horse out for a ride. If they bolt back to the barn, you turn around and go back to the spot they bolted. Yup, one time it took me 1.5 hours to get back to the barn! *laugh*
Luck!

5 weeks is too young for a puppy to be away from its' mother.
Jim has a good suggestion. If this happens often, keep the leash on your dog all the time and let him drag it around. Whenever someone tries to open the door, hold the leash so he doesn't go out the door.
I'm sure some other people will be able to give you great advice and links to some useful articles. :P

with my dog from day one I told him to sit before I opened the door (OK, as soon as he knew what sit was, which was fast). He would have to hold that sit for as long as was possible (1 sec, 2 sec., etc.) increasing the time with his age. I always walked out first then would give him the "OK" to come out, slowly increasing the distance I would go first. I can now put him in a sit and walk out to the car to get something with the door open and he waits, even if I go out of his sight. He is 8 mo. old.
This can be a hard habit to break once they have it. The advice to use a leach on him at all times for a while is good. Make sure you have him sit before you open the door. practice it with a friend coming to the door so you can control the situation, tons of praise for good behavior. Good luck!

Do you have a runner rug by the door? Or some physical dividing line like a hallway, stairs etc? If so it is a pretty easy solution to train and proof.
Use a 6-8ft line and a prong and set up door scenarios. You need someone to keep going to the door and entering and someone behind the dog holding on to the line. Anytime your dog goes on to the dividing line you don't want it to cross they get a correction. After a few zings of the prong trust me the dog will get the message then it will be only voice corrections and then become automatic. Remember to treat the dog and praise the dog when it does the behaviour right.
Make it so the dog has to be invited to the door and has to wait all the time before going outside doesn't matter if there is someone at the door or not.

A method I learned from Leerburg is to only open the door just enough for your dog to fit through. As he tries to bolt close the door once his head has gone through just enough to not allow him out or back in. After a few seconds let him back in and try again. My dogs, and my mother-in-laws dogs who never listen to her, learned VERY quickly not to push past me at the door. Take care not to hurt the dog, only apply enough pressure to trap him. And also go with the sit before exiting as mentioned above.

Just an FYI, the above method outlined by Eric does NOT work on the refrigerator door. Other doors, yes.

Yes, your persistance and consistancy in this training will most definately pay off. It takes some getting used to, but once trained, it is very nice. Door wide open and dog is sitting inside waiting your release.
