intermittent lameness

The dog I am babysitting goes lame sometimes. Owner has had xrays, numerous vet exams etc. they've found nothing wrong. He does have a 'lick granuloma' on that wrist joint and I'm wondering if the two could be related.
He limps from the front right wrist, sometimes picking that paw off the ground as though he doesn't want to put weight on it.
Today he started limping after our walk, I had him on lead for maybe ten minutes, maybe fifteen minutes off lead and then ten minutes on lead - I wanted to warm up/down to avoid this but still went lame. He is used to that much/more exerscise so I didn't overdo it - no chasing the ball or anything.
Any ideas?

He's a rescue, estimated about 5 - he's a gsd.

IME lick granulomas occur as a displacement activity-the dog is worried about something else, pain somehere else that they can't reach (hip, anal area) or separation anxiety, that sort of thing, they begin working on a single area, hair is denuded, viscous cycle scenario. I have not seen many dogs lame from the lick granuloma itself.
Perhaps the dog has soft tissue illness, nerves of the forelimb, rotator cuff injury, something like this which can't be seen on a radiograph. The wrist does appear swollen, it's looks sore and very chronic.

I'll pass that on Betty, thank you - I think it's time they got a 2nd opinion too.
He does have mild HD too, they're great about managing it - have known him years and he doesn't seem in pain from that.
That said he is a very very nervous, stressed dog. Today was the first day I left the muzzle off when I put cream on his leg, I hoped that he wouldn't tear my face off and I was right.

Oh, boy, limited exercise on top of being nervous and stressed ... chicken or egg.
Yes, 2nd opinion, yes. He needs to be rehabbed and then to be exercised. JMO.
How long do you have him? Does he gets lots of training to at least work out his doggy brain?

Well he gets about 30 mins AM, about 45 mins lunchtime, creative 'in-house' games during the afternoon and two 15 min walks in the evening.
I stayed with them for a few days before the owners left, They don't come back until next monday.
His general obedience is very good but he is very dog aggressive which reduces the time he can get off lead. He is very stressed but tons better than when I first me him 3 years ago. Poor lad.

Suggest they take him to a chiropractor. There are things that can be "off" that an x-ray won't show. Some vets are very good at pinpointing "ouchie" areas through exam, some aren't as good. I don't know if their's is or isn't, but a second opinion is never a bad thing, especially when the first opinion can't find anything wrong, and a chiropractor is used to feeling for sore areas just using their hands and the dog's reactions.
The lick granuloma might be related, or might not be, might also just be coincidence and the result of him being overly anxious.

I think I will tell them to do that, there are plenty of dog specialists around and he is insured so they've nothing to loose. They're moving a few days after they get home so they'll have a new vet in any case.
Biggest problem with this particular dog is examining him in any detail, I think last time his leg was investigated he had to be heavily sedated. He isn't the friendliest dog.
