I hope it's just starting, tiny, and that the dog can be distracted from it.
I know that there are many many possible triggers, including small skin problems or pain in a certain spot, but the most common ones (I believe) that I've seen have been related to boredom/loneliness/S/A. (That is, the small skin problem or pain is not usually the whole trigger. Most dogs even with such an irritation or pain do not develop acral lick dermatitis, and many dogs with no initial skin problem or apparent pain DO develop acral lick dermatitis.)
This is only my observations, and I'm not a health professional.
I know too that there are breeds more prone to this, and these breeds (I believe) tend to be also prone to OCB in general.
I found a good overview of this once, and I'll dig it up.
For me, the strongest advice, even before trying to determine the actual trigger, is to stop it with activity. It's as urgent as stopping tail-chasing, I believe. ASAP.

My female has had lick granulomas on a couple occasions.
I have had it treated with cold laser treatments and that worked really well to heal it.
I have also used Vetericyn on the spot, to kill germs etc.
It works well too but you have to keep the dog from continually licking the spot repeatedly in order for it to heal.
I had to briefly wrap it with vetwrap to stop the cycle. Wrapping loosely,for short period, taking it off to let it breathe.
I recommend the cold laser if you have been unsuccessful in getting it to heal on your own.
Having tried both, I do believe this method heals the fastest.
These are just my experiences.
ETA; my dog has severe inhalant allergies and I wonder if this is connected, during some of her really bad cycles of itching?

My Pinker was stepped on by a cow, wore a cast for 3 days, had an xray that showed the fracture was just the metacarpals, cast could come off, though foot was pretty "smashed". He was anaesthetized for the Xray.
As he came up out of the anaesthesia, he created a lick granuloma on his wrist. I've had it wrapped, he has an incredibly full life, more full a life a dog can't have. I even let him sleep on my bed last nite (he's never been upstairs before -hubby is at a conference....)
Now he's wearing a soft collar.
So I know the medicine--- but I just need tips on managment, things likely folks who have delt with this know about, and I don't.

Oh, I had no idea it was Pinker!! The poor lil guy!
I certainly did not mean him for boredom, S/A, etc. Obviously, he had classic physical triggers, the poor guy.
Does the soft collar work? He can't reach it now?

He is wearing hubby's airline neck pillow which is quite heavy. He can't herd.
He can't reach it. Several times I have made the mistake of vetwrapping the lesion and having him help me with the sheep (we just bought 10 ewes and they are wild) but as soon as I look away he has the wrap off. So now I'm just keeping him inside.
It's a real bore but the accident with the cow could have been much worse. I'm grateful it wasn't worse.

Yes. As bad as this is on him, it could have been a tragedy.
