I have tried tethering, but she is so fast to pee sometimes, I can't get her outside fast enough. And, I am using about a 2-3 ft tether. Help!

I wouldn't give up on the tehtering idea (the best and quickest way to housetrain, IMO), but I will say that a dog that is so quick to pee when she's tethered to you with only 2-3 feet of line is a dog that I would have evaluated by a vet to rule out any medical conditions.
A healthy 5 month old pup should be able to hold it long enough for you to get her outside if you don't dilly-dally. Does she make any sort of indications while she's on the tether? It doesn't have to be whining...sometimes it's just restlessness, or suddenly seeking attention from you.
And again, have you made sure that you have cleaned the areas she's peed before with a solution like Nature's Miracle?
Do you use a crate? Is she able to hold it in there?

Yeah, Amber is right. Get her checked for a possible bladder or UTI.
If she comes back with a clean bill of health, I'd bust her back to the 2 - 3 month old rules. Crate her unless you're actually interacting with her, since it seems like she has no incentive not to go in the house. Use that crate for 1 week, work the schedule, and she should be ok.
What's her situation when you're at work? Kennel? She's too young to be crated for 9 hours. Does she pee in her crate? The more structured and hands on you are, the faster the housetraining process will go.

I will get her checked for a UTI. She can hold it throughout the night no problem. She is almost 5 months old and can go for about 4-5 hours in the crate at night.
I work at home, and when she is not tethered to me, she is in her crate. I take her out to go pee about every 45 minutes to an hour, and she responds quickly to the "get Busy" command. She never poops in the house, and always goes in the same spot outside.
She has never been in the crate more than about 5 hours. She has peed in the crate twice, but that was about a month ago. Also, it was my fault because we went for a walk and she drank a lot of water when she got back. I don't she has gotten comfortable with peeing in the crate.

Spoke to soon, but I think it was my fault. She was in her crate and needed to go out at about 1:45 am. I let her out but she pee'd at the back door. I picked her up and put her outside, and she had quite a bit left over. I am going to try being a little more strict on letting her out of the kennel. is this common?
At what age will she be less prone to the occasional accident. She is 6 months old now, so I suppose she is really still a puppy. Thanks for your suggestions.

Oh yeah .... I have a small senior with a teeny bladder who needs out often, and once in a while I forget, when I'm not in the room with him (where he will stare at me with an urgent stare). Then he will get as close to outside as he can, leaving a puddle right by the back door.
You're right; she just couldn't hold it all the way outside.
What was the part about strict out of the kennel? I didn't follow that.

Well, it's really more of an environmental thing, IMO. Of course, a bigger bladder and stronger bladder control muscles will make a difference, but IME with adopted adults, the big thing is when the dog understands that he will be relieved.... that he can hold it for the little time he has to, because he WILL be brought outside. Having adopted dogs who had never been indoors, I'm convinced that once the dog "gets it" about not just letting go wherever the urge takes him, then reliable schedules are what encourage the dog to hang on.
The less the dog has to have an accident because of poor timing on my part, the better, because then the outdoor potty habit becomes a strong, second-nature thing to the dog.
