the gland thing is not hard, and easy to determine whether its within your skills level. I warm and relax the dogs butt, then aim your fingers on both sides of the anus at two and seven O'Clock. You will feel the glands under the anus, and the idea is to apply pressure while slightly pulling outward. Keep your head out of the way, and with any luck you will have a palm of fishy smell. If not, then its time to allow the vet to do it.
Also, when this occurs, I try and start spirulina and other herbal fungus/yeast killing supplements. I find that giving any type of antibiotic while the animal has a yeast or fungus issue, exacerbates the problem.
