Her paws are not red. She does scratch her neck a lot. I feed her the Tyson chicken out of a can. Is that bad? I will switch her to plain yogurt.
Tyson canned chicken for people? Salted? I'm not familiar with it, but if it's for humans then it is not phosphorous-calcium balanced for a dog, and I would stop that asap.
Is this a small addition to the grain-heavy food you are giving, or is it, say, half the diet?

At what age did the ear infections start?
Did the first one coincide with anything out of the ordinary (illness, antibiotics, a vaccination)?

A little chicken is mixed in with her kibble. She started having ear infections at around 4 months old. Very soon after her last puppu vaccination.

Oh, this reminds me that I meant to post a reply to a question about giving yogurt to allergic dogs. Jennifer, was that your question? Could you PM me a link if it was?
Sorry .... my sticky notes got away from me......
If I am the Jennifer you mean, no it wasn't my question, but I would like to know too
I personally have always fed my allergic dog, all of them actually, either yogurt or kefir nearly daily and have only seen good things from it.
The other thing I wanted to mention to the OP is Zymox drops. They won't solve the underlying problem but seem to work great for ear infections in conjunction with treatment from the Vet. I have used them both with and without the cortisone and have seen great results from both products.
My vet said they are safe to use with oral medications and he sees good results with them too.
You can get them at Amazon, Drs. Foster and Smith and at some local pet stores.

Gotcha.
Well, number one for me would be a much better food, and no added salty muscle meat like this http://www.tyson.com/Consumer/Products/ViewProduct.aspx?id=152
There's more than salt to think about; there's the muscle meat unbalanced with calcium in a boneless meat meant for people. "A little mixed in" does not sound like a big deal, but it's still not a great idea. JMO.

She does have two circles on the bottom og her muzzle with hair loss. Also her genetials are red and there is a rash.

1. Has there ever been a scraping done, when the vet lifts off some skin cells (kind of like with sticky tape) and looks at the result under the microscope?
2. Is there any odor to any part of the dog, particularly the ears and the rash areas?
3. How long have the rash and hairless spots been evident?
4, What does the rash look like? Bumpy, or plain red, or what? Is it weeping or damp?
5. If the rash was there when the meds were being given, did it improve and come back, or not improve at all?
6. Is any of her skin waxy or greasy feeling? Is there any flaking? Is any skin darkening or thickening?
If you can answer all the questions at once, that will be much easier than one per post -- so we don't forget any.

Also, it is hard for us to get anything in her ears - she hates it! If I can wrestle my 120 lb Great Dane puppy into a position for ear treatment, you can do it with your GSD. She won't like it, but she will like less having one of these reoccurring ear infections causing permanent damage. If you have to, use a second person to hold her on her side while you manage the head. Your the master/mommy, you set the rules, you always win when you set your mind to it. You're dog needs to be manageable by you in an emergency, so start with these little routine medical things.
Our vet recommended twice a week cleanings after getting the initial infection cleared up with prescription medication and daily cleanings.
You might get a bit less resistance out of her if you pre-warm the cleaning fluid. Put the bottle inside a plastic bag to keep the label dry, and then put that bag inside a cup or bowl of warm water for 5 minutes before doing the ear wash.
