Raw food won't make him worse. The biggest thing I noticed about your post was that they want you to dip him. I don't know for sure, but I would think that those harsh chemicals cannot possibly be good for such a sick boy. I'd concentrate on boosting his overall health and immune system via food, so find out asap if he's diabetic or not. THEN, when he's generally stronger, you can reevaluate those dips.
I was thinking this, too. The authoritative mange web sites do say that treatment is needed in a case this bad, but on the other hand, this may be a situation where all you can do is give palliative, or hospice, care, and give him a few days of care and good food and comfort before PTS.
I'd want to talk to the vet if I were you, because s/he will have a better (less emotional) answer as to whether it's worth putting him through the harsh treatment.
As you know, a gentle end with a caring person nearby is sometimes the most generous and kindly gift you can give.

If he's that underweight, try ground beef-higher in fat. Or chicken/turkey w/skin, although you'd need to be very careful when introducing it. The richness/fat can cause stomach issues, and we certainly don't need him getting diarrhea or vomiting on top of all this; it could wipe him out.

Yes, I agree that you don't want to start with fatty foods..... that needs to be gradual.
I would start with ground up white poultry and see how it goes for a day or two.
Don't forget the liquid fish oil and Vitamin E gelcaps. No matter what, these are important to any dog; they won't be wasted.

ok will go with ground white turkey or chicken doesnt matter ?? or should I go with beef or both ????will get e-caps too, got fish oil (salmon)
I know, I may lose him, it has been too long and this is going to take a miracle.....I will know more tomorrow about meds, dips they are using and any other things you need to know..
should I use a topical for him to soothe or heal??What is your recommendation ???I have seen the neem oil recommendation, dont know where to find in Waco, tx though

I agree, I think that dip is too strong for his skin,,,girl, he is ATE UP with it.....no, no honest kitchen left what else should i pick up to replace that for now ??
WIll also get ground turkey, got salmon oil left and will get plain plain yogurt too and the liver
Make sure your salmon oil is not rancid (sniff it) and be sure to get Vitamin E (400 IU will be good). Don't give rancid oil.
OK, you can pick up some baby food for his beginner ingredients. I'd try the baby food green beans and the squash. You can also pick up a couple of summer squashes (the baby food is winter squash, more like Hubbard) and grate them or chop them up and lightly cook them.
I'm thinking of palatibility, because it sounds like he's not eating well. I'd be wanting to give him something he will love, and IME dogs love ground poultry, orange squashes, and lightly cooked summer squashes. I have also never met a dog who didn't like baby food vegetables. (Be careful that it's not "and rice.")

He probably has lotion prescribed. I'd call the rescue and check. Turkey or chicken doesn't matter.
If he has no topical stuff, post back.
BTW, if he is refusing to eat when you offer the fresh food, then
I'd be thinking that it's time to make the PTS decision. I'm sorry. (First, of course, you can try lightly cooking the surface to add to the flavor; try the raw first, though, IMO.)

ok oil smells ok due date is this month though,,,,will get baby food, get green beans and squash, white poultry more fresh oil....and keep you posted, thank you all, so very much I really appreciate all of the effort you are putting into helping me on this one...When I get, do you want to see photo of Mac, or you think too much for you ladies ???

ok Connie gotcha,,,,I think he may still be eating...and i bet he will love it when he gets to my house, coz of all the great food he will be getting vs the nasty dry kibble with corn probably that is NOT helping him..I love you ladies !!!!
I havent seen them med the dogs,,,,,what if no topical, what do you recommend i get ?? I want to have all things possible first day

So, call the rescue: Has he had blood tests and does he have diabetes; does he have topical lotion?
Then check the fish oil for freshness.
At the store, pick up Vitamin E gelcaps (and fish oil if yours has gone rancid), ground low-fat poultry, plain live-culture yogurt (like Cascade or Stonyfield or Clover ... any of the active culture brands), some baby food vegetables and/or a couple of summer squashes, therapeutic Neem oil if he has no topical stuff from the vet.
