OK....here is the info that I just got from a call to the pharmacy that produces my Rx. This is diectly from the pharmicist at the compounding pharmacy that my Rx comes from:
It is a natural plant derrived hydrocortisone. It is a soy/yam based non-synthesized hydrocotisone. It has the effectiveness of prednisone but has a 1/2 life & doesn't last as long in the body & therefore doesn't have nare the side effects of prednisone.
Lori: He didn't have any idea what was meant by " Natural Hydrocortisone is bio-identical cortisone".
Hope this helps & clears this up.
ETA....although there may be some limited soy is this...my dog has no problems with it. I feed raw & do not feed any grains & VERY limited dog treats using zukes & dryed liver biscotti treats that do not contain wheat, corn or soy. Dogs that are more sensitive to soy may not do well on this Rx.
Jeezzeee Lori...this all beats the hell out of me. I have no chemistry background...so I don't know. Maybe the 'cortisone' that my dog is on is different then the one your vet talks about.
All I know is that it works very well for my dog & she has no problems with the 'ingreedients' in the compound. So I am grateful for that.
Update on my GSD. Made appointment with dermatologist but had to wait 2 weeks. In the interim, another sore developed on the back of her wrist. Went to our primary vet who did a scraping, found no infection, bacteria or parasites. She did laser therapy three consecutive days, had me switch from Tritop ointment to Gentamycin spray, and continue with the hydroxyzine. Seemed to help. Saw the dermatologist yesterday. Confirmed what primary vet found. Mentioned that because my girl has had allergies since she was young, they get worse as she ages. Derm doc advised continuing with my primary vet’s recommendations and if improvement seemed too slow, go back for additional laser therapy. Both vets also noted that our extremely hot, dry summer had a lot to do with the problem; that many people were asking how to stop their dogs’ biting, scratching and licking. So I am continuing as directed and seeing steady improvement. She has done no harrassing for the past 2 days. Weather is cooler now; maybe that helps.

Diagnosis was mild dermatitis, allergies. So we continue with Gentamycin and hydroxyzine. There is significant improvement so we may not need to have more laser therapy.
