No gnats. The dogs are inside with me. I've never noticed them outside either. Yeah, derm vet here I come. Sarcoptic isn't out of the question since he had no anti-mite stuff from April 20-May 20 and the trail we walk on almost daily has foxes. Sarcoptic is possible. Like sarcoptic + fleas. Yeah, I'm nicely demanding to see derm vet Monday. :-)
Believe it or not, his ears are better than they were in Jan.
Sarcoptic mange is mega-itchy. Some ringworm is itchy too. Did anyone ever put a Woods light on your dog, a blacklight?
Sarcoptic+fleas is a possibility, though USUALLY problems are one thing. Allergy will have to be ruled out.
Endocrine things will have to be ruled out too.
No blacklight ever No one has really looked hard at allergies. Just "oh maybe it's food" but nothing scientific. And endocrine has never been mentioned. Feeling rather glad this is the dog I have insurance on...
And thank all of you for your replies. Aside from the information, it's also vastly relieving to know I am not the first to go through something like this and to have a good (and reputable) place to talk about it.

Cost aside (and my experience with derm issues .... a lot of experience, sadly ... is that the derm vet saves money and misery, despite the sticker shock; nine months later, you would not be in this situation), the dog is itchy and losing sleep (and fur and skin) and you are frustrated.

With all my dog skin experience, I'd be at the derma vet yesterday! This skin stuff can send you to the insanosylum if you keep trying to figure it out without one!!
Well I looked one up and the one my vet practice recommended is the same one a friend just went to and said great things about, so I feel good about calling tomorrow. I've been videologging his scratching for them..fun fun. I will let you all know how it turns out.
