It's not OK to feed raw with kibble (in the same meal), and it's also not good, in general, to feed muscle meat with no bones.
Kibble takes a long time to digest and adding raw to it means that raw food spends an unnaturally long time in the dog's system, losing the dog's inherent ability to deal with microorganisms in raw, which has to do with natural speed in getting the raw from one end to the other.
Cooked hamburger: Adding muscle meat (without the appropriate RMBs) as a regular thing, even cooked or at different meals, throws off the calcium (bones) and phosphorous (meat) ratio dogs need.
How underweight is he? Isn't he small, rather than underweight?
Do his ribs all (or mostly all) show? Does his vet think he's underweight?
Have you considered adding fat in the form of salmon oil or RMBs plus meat, IF the dog is actually underweight rather than just small?
I have done this a few times with underweight dogs, or dogs that are hard to maintain a good weight. Never did mine a bit of harm and they re-gained weight nicely.
I think a a general rule most 6 month old pups are over weight. It could be a blessing that yours is thin. I keep mine on what most people think is under weight, but they grow much slower and sounder because of it. You will have few growth related problems to keep your pup a little hungry all the time. As an added bonus it makes them very eager to do puppy school for food rewards. I would not feed meat with out bones on a regular basis.
I, too have mixed ground meats(turkey and beef) with a high quality kibble with no problems. Adding cooked brown rice(about 1/2 cup) will also help with weight gain.

Matt, with your pup's health history, I really would avoid mixing raw with kibble.
I feed raw, but I wouldn't mix it with kibble because the kibble forces the raw food into unnaturally long contact with the dog's system. It probably causes no problems most of the time with most dogs, but your pup doesn't need the risk. <img src="http://www.leerburg.com/ubbthreads/images/graemlins/wink.gif" alt="" /> JMHO.
Maybe you meant cooked meat.
Either way, just be aware that the regular addition of meat without bones will change the calcium-phosphorous balance. If you decide you want to know how to add meat without bones and correct the ratio for it, I can give you some eggshell/bone powder info.
P.S. If the vet doesn't think he's underweight, then, as Doreen says, lean is better than chubby. JMO! :>
