I went through this with my GSD. When I got him from my sister, she was using "piddle pads". Bad idea. I removed all bedding from his crate because he thought all bedding were piddle pads.
The other mistake I made was I thought I was doing him a favor by getting him a ex-large crate. Another bad idea. He'd pee on one side and sleep on the other, so I put up a partition in the crate to give him a smaller area.
I closely monitored his water intake. 15 minutes after he drank, I took him out to the yard. (same door everytime, same area of the yard) or after he woke up from his nap, I took him out immediately.
He started holding it all night at 3 months. Before that I was getting up at 3:30 AM to take him out. I sympathize - but this too shall pass.

Thanks for all the good advice. Last night we fed him earlier, took away the water at night, covered his crate, and I got him a puppy Kong to play with in his crate and he did beautifully last night and was very calm and playful today. It helped that we were home today, so he only had to spend an hour or so in his crate all day. The rest of the time, he was playing in the yard with the kids, snoozing in my lap or on his leash in the house. He is getting very good with his elimination word - he has had about six times when he has gone right on command. He's a smart little guy and we're really enjoying him.
