This site lists animal incidences with the airlines. Most of the problems were predictable: snub nose dogs stressing and dying, cats escaping from their carrier, over-sedated, stressed old dog dying, terrier getting it's teeth caught in the door.

Being able to do this is such a privelege that most SAR folks I know do NOT take it lightly and do NOT invoke it for personal uses.
There is so much that CAN go wrong taking a dog into an airport environment ..... an incindent several years ago with a "therapy pig" - yes, "pig" almost took away that privelege.
Slick white floors, noises, mobs of people and uncontrolled kids, being scanned by a stranger with a metal detector, confined spaces, diesel smells, traffic, elevators, subways,etc. .... oh and on one trip I was seated next to a Saudi - he was very nice but dogs are "unclean" so I spent the whole time making sure my dog did not touch any part of his body.
