my 13 week old pup had a UTI which was causing him to wet his crate. I took care of the Uti and BAMM no more crate wetting...until today I left him out at 12:45 and he went outside. When I came home at 5 he went pee in his crate. What should I do???

There is nothing to do after you find it.... There is something to do only when you catch him in the act and can take him out, to praise him for finishing outdoors.
I remember this pup's UTI..... I would suggest that a new urine spec be collected and checked, because it does sometimes happen that the antibiotic protocol did not really completely wipe out the infection, which gets going again after the regimen.
But as Carol says, it also could easily have just been too long for his bladder.
You are making a lot of progress. You kept a log and discovered the frequency that made you think UTI. You addressed that. A tiny setback at this age, whether it's just a step back in the frequancy of taking him out and praising, or a recurring infection, or just overestimating his bladder size -- these can all be fixed.
I am not sure why vets don't routinely do a second urinalysis after the med protocol to find out whether the infection was really gone.
you guys are both right I found out today that someone was knocking on my door for about five mins. I was also told that both of my dogs were barking like crazy.I wonder if he just got excited!!!!!
