Yes, it certainly is. You can't take out a piece of paper and replace it with a different one, either.

Regarding your statement about "breach of doctor/patient confidentiality": Only a handful of states protect the vet-patient relationship with any law. Indeed, California is one of those few states. However, James' profile does not indicate the state in which he lives. In the vast majority of states, there is no such protection. Unfortunately, most states view dogs as "mere" property. As such, priveleges do not apply.
So, my recommendation stands. It never hurts to ask or try. James should find out all he can, right now. Evidence gets stale, people move, etc. When asking those questions, I would, at the very least, have someone present to witness the answers.
And while it may be illegal to alter medical and/or vet files, critical pages all too often go "missing." Connie is absolutely correct in her suggestion.
The vet told me it looks like the dog may have vommited, it does not say it in the chart or the surgery log. She did not do the surgery. The vet that did is now on vacation.
I informed her in frank manner that i am very unhappy with the situation i find my dog in. I asked her if they were able to cover some of the cost. She would check with the other vet who happens to be the owner. She did say this can happen, dogs
can have things left in there stomach before surgery, and not throw them up completly, so the Dr or Tech would not see it.
