The same thing can happen with a kong.
If you use too small of a piece of kibble or stuff the kong with something soft like cream cheese or cheez whiz it will effectively block the second hole on the kong, and will create the same kind of vacuum seal. It happened with my other dog, I had put in cream cheese first not even thinking it could clog that little hole and after about 10 minutes of Taz licking inside it, his tongue had forced down the cream cheese so that it blocked the hole. It did get stuck on his tongue but I was there (thankfully) to just stick a pen through the little hole and his tongue came out.
I now put a large piece of biscuit in the Kong first then other treats then cream cheese. Just in case.
