I hope you will get another response that is more specific, but getting the dog off dry food at all will definitely help. Raw food is best but even canned food contains the vital moisture required by kidney patients. Whatever you do, don't feed the kd food, it's nothing but dried cereal and will only stress the dog's body even more.
I had a cat die of kidney failure. I made every mistake in the book. One of the things I wish I had done was have his blood tested regularly and make a spreadsheet with the exact values of BUN, creatine levels and keep track. Regular urinalysis should also be done.

sue, that is great to hear, that he has a great home!
You will definitely need to write down his results and keep track. Since you have been feeding him a great diet I am certain you will see an improvement over the coming months. Depending on how severe his kidney disease is, you might also have to give him subcutaneous fluids (inject fluids under his skin) - as hydration is a problem with kidney patients. Your vet will help you with this and tell you when it is time for this (my cat survived a year with sub-q treatments, he was 16 years old, so I don't think you will have to do this for a long while). Just remember to warm up the fluids to body temp in a sink of hot water, as it would be very unpleasant to get cold fluid injected in your skin!
This site will help you with gearing your raw diet to your kidney patient.

I had just PM'd those two links when I saw this thread.
As I mentioned to Sue, I consider Lew Olson and Mary Strauss (those links) to be the two best canine nutrition sources on the 'net.
Mary Strauss, of course, is the nutrition researcher/writer for Whole Dog Journal.
I agree 100% with Angela about not giving dry food to a canine kidney patient. I would probably pretty much take that advice as a sign that the vet who recommended it got his/her nutrition education from Hills.
