Chewing can be extremely painful with an ear infection. And antibiotics can trigger nausea. It might be a juggle to find the right time to feed and dose. If you can give a soft small meal early on and then give the antibiotic on a not-empty stomach, that might help with both issues. Did the vet say without food? Giving with food can delay the absorption slightly, but from what I have read, it does not affect total absorption and is probably worth doing when you see GI distress.
Tramadol is a pain med, BTW. I use and prefer it over many others, but I don't really see it as a calmative. I have read the lit saying that it can have that effect on some dogs, but I'd look to it more as a pain med.
