If a Border Patrol Agent were to ask me that, my reply would be "Sir, my health issues are none of your business."
Jeff, although it isn't 'anyone's business' but their own, the border patrol still has a right (and probably obligation) to ask, especially if the person is bringing prescription meds in. Refusal to answer or deception will likely get you banned from entering the US permanently, or force you to apply for an expensive waiver.

You don't have to have a visa to VISIT the US. Just to reside here.
You may have to have a visa (ie my m-in-law from Mexico has a 10 year visitor's visa) to visit, depending on what country you are from, or you may qualify for the VWP (visa waiver program) if you reside in a country that participates in that program.
Canadians don't need a visa to visit, but from what I could glean from the DOS website (and others), especially if they are bringing meds in, they will have to endure questions from border patrol regarding their health.
