Whether you see flea dirt or not, you are describing classic flea itching (backbone and base of tail). ("she try to nap nd itch on her back spine d place connected wd her tail,")
Also, even if the vet you have access to doesn't meet your expectations, he or she can absolutely tell you what his (I'll say he for convenience) diagnosis was when he recommended ivermectin.
You're in a very different geographical are from many (or probably most) of the board members here, so we can't rely so much on our experience to answer some of your questions. We need hints: What is the vet saying to you? If you got no answer at the time, call and get it now, would be my advice. Help us help you.
She is one year old? How is her body condition? You mentioned that she has almost no appetite ... is she losing weight? Did the vet give any ideas for why she has no appetite?
Is she basically healthy-acting? Playful, energetic, sleeps well? Or lethargic, lying around, etc.?
P.S. This isn't the dog to be bred soon, right?

In the spring when my two GSDs shed my back yard looks like dead bunnies rolling around in the breeze.
I can fill a trash can with hair 3-4 times a week for most of a month before it stops.
With a one yr old GSD your possibly seeing the first time shedding of a winter coat.

Two cents... If the weather changed by winter, summer degrees, and around CNY that swing happens within days and ramps up and down by as much as 50 degrees, then in my mind it would be the annual shedding of the winter coat.
If there was evidence of fleas or the like, then I'd be administering monthly flea tick stuff.
If there are spots where the fur is gone or all but gone then I would suspect a topical or internal medication might be appropriate.
But when all is said and done, a competent veterinarian ought to be able to shed light on the issue. Shedding coat and checking for fleas, and the like, is the first step, it seems to me.
My five are dumping hair at what seems to be a never ending rate. Hair, hair,
Gimme head with hair
Long beautiful hair
Shining, gleaming,
Streaming, flaxen, waxen
Give me down to there hair
Shoulder length or longer
Here baby, there mama
Everywhere daddy daddy
Hair, hair, hair, hair, hair, hair, hair
Flow it, show it
Long as God can grow it
My dogs hair
Where did that come from?
I know, way off the mark of this thread but I chalk it up to not enough coffee.

i jst saw ur reply after getting bck frm wrk,its late night here so i better call my vet tomorrow to talk wd him what u jst asked, btw she s still vry playful, energetic, she sleeps well too,no signs of lathergy..btw let me tell u its summer in here ,most of d days goin near around 38 to 45 degree centigred tempureture...it could be d reason too dt she s nt eating properly nd getting her hair loosing..
p.s if her health s nt good,dr s no question abt breeding,m nt gonna breed her untill she s in condition, although i m thinking to breed her nt early than her third heat circle, she only had one circle whn she was 9 months old,now she is almost 13months old
