Likely it's the antibiotics plus the general stress of the whole deal.
Small meals. Boiled rice and chicken. Consider dropping the antibiotics if he's a few days out from surgery. Post op infection is very rare, though post-op irritation around the scrotum/incision site is common.
Gosh, a great dane with fire hose diarrhea and urinary incontinence. That's unmanagable for most folks.
If he's on antibiotics they can cause some diarrhea.
I'd fast him for several hours before giving him a few small meals of very bland foods. Boiled chicken, egg, overcooked(mushy) rice along with some pumpkin. He'll be hungry but his gut needs some time to settle down.
Here's another procedure that was introduced in 2011:

Unfortunately some of the procedures are only able to be used on females for spay incontinence but I don't know if that's because it's the only way they studied it or if there's an anatomical trait that makes it ineffective for male dogs.

Yes he was fasted for 10 hours then the typical boiled chicken and rice. That is all he has been fed in the past 24 hours. He has not been on any medication for nearly four days now. So I'm unsure if the absense of the medication could cause gastrointestinal issues.

To clarify: He's still having completely liquid diarrhea after a fast and 24 hours of bland foods?
Is he acting sick? Fever? I'd be thinking of adding an anti-diarrheal but would want to talk to the vet first even if it's just a phone call. In some situations anti-diarrheals can harm more than help.
He is having less frequent bowel movements. However they still aren't formed. Not as bad as yesturday, but still soft. Thats progress in my books! They added in some pumpkin today, perhaps thats helping?

He has urinated once, however his bowel movements are back to normal. Vet instructed to continue with the pumpkin until his bowel movements are in check. So all is good and well in that department.
