Is there any realism added to the protection phase of the Bundeseiger or other Schutzhund national competitions? I'm curious as to whether the helpers in these higher class areas are more demanding on the dogs, testing their defense drive. From what I've seen, the protection phase appears the same(all prey); the helpers are just more skillful.

As in the protection critique: “Hardness, courage and fighting instincts pronounced”. And of course the helpers are (or should be) more demanding at high-level trials. You noticed that they are more skillful. Part of the skill is to be able put pressure on the dog without it being obvious to the audience.
The routine is the same in the Bundensieger as a club trial. So there will not be any big difference of what is going to happen as far as the routine.
But there are some differences at the Bundensieger. The first thing is the stadium. This alone will test a weaker dogs nerves. The sounds and people that fill these areas is sometimes a big disraction to a dog that has nerve issues. The judging is suppose to be more by the book, and strict. What may loose you a point in a club trial could cost you 3 or 4 in the Bundensieger. For example a dog with a less then full grip might get docked a little in a club trial, but will loose a bit more in a National level trial. The helpers are also suppose to be really pressuring the dogs harder (the drives should be long and fast with solid stick hits); thus really testing the dogs nerves and fighting instinct.
