I expect someone has experience with registering a dog with SV papers with AKC. I have a couple of questions: 1. the AKC application says to 'attach a German certificate of transfer in letter form if applicable'- does this apply to all dogs from Germany? I don't have such a letter and wonder whether the application can be processed without it? 2. 'The AKC application and the foreign registration must reflect identical ownership'. I don't see a place for the former owner on the AKC form, only the breeder. Am I missing something here?

You need to sign the back of the SV pedigree and send the stuff to the SV. They will record the transfer. You need to request a "to whom it may concern" letter from them stating you are the owner of record for the dog.
Then take a couple photos of your dog and fill out the green paperwork.
