I'm not a vet, this is personal experience/observation
That looks JUST like the hair loss a dog in our complex had after rolling in some pesticide sprayed area (most of it had dried, but they hadn't cleared the area). It got on her back and she lost a patch there, then when she laid down in the grass (on her belly) she got some there, the hair was gone the next day, even though she got a bath (and vet visit)

Daniel, I'd want the vet to see it before putting anything on it so if s/he needs to do a scraping (lifting off a few skin cells to look at under a 'scope), it will be unaltered.
I strongly recommend not mixing raw with kibble (although I don't see it as related to the rash or anything; I'm mentioning it because I saw it and feel strongly that they should not be mixed).
The only dogs that cannot (should not) take Ivermectin are collies, and any kind of mix that potentially has collie, BC, Sheltie, or Old English Sheepdog. The Collies have a potential genetic weakness to ivermectin, and to my understanding it can cause death. A few horse owners I've heard of would give the leftover equine wormer to their dogs, and then have a dead dog. Since Xena is a shep, you'll be fine!
I don't think Ivermectin is a approved tx for demodex. The Tx regimen that I am familiar with involves medicated shampoos, and insecticides to kill the mite. My vet also told me that Interceptor can be used. (It's a heartworm preventative)
I do know that some pups clear up on their own, and I believe the % is around 50% in puppies under 7 months. You may want to get a second opinion before treating her, Ivermectin, and the 'normal' Tx (medicated shampoo, insecticide) is powerful stuff.
There is discussion and citations about the difference between juvenile (under age 2) and adult demodex.
"Adult dogs who get demodex generally have an underlying immune system problem that is letting it overrun on the body. Localized demodex in puppies is different .... "
