Tammy- thought I'd throw this out there, but you might want to consider tick paralysis? I've heard it can have similar (deadly) symptoms to what you describe. It does occur occasionally in the Northeast- and other parts of the country. If this is what it is they need to find the tick and get it off or the dog will continue to get worse. Recovery is quick after tick removal.
I know this is a long shot. I'm so sorry to hear about this. My last dog went down fast with seizures/neurological signs. It was heartbreaking, and I honestly waited too long before I let him go. One thing to keep in mind is whether an expensive nuerological evaluation/cat scan/MRI would change the outcome for the dog. Would they do brain surgery? Would it be fair to the dog?
I had a hard time not doing everything possible for my boy, but in the end I had to keep realistic about whether tests would actually help. I know the bills add up fast.
I know I keep beating the tick illness drum, but my shepherd came back from a run one day with serious head tilt, dizzyness, disorientation (nystagmus didn't set in until 24 hours later). Turns out it was *probably Rocky Mountain Spotted fever, and after 6 weeks of doxycycline she's back to normal- and the head tilt is indiscernable if there at all. The vet was stumped at first. She suggested brain tumor, and a nuerologicial evaluation. There is a chance it could be something like this for your brother's shepherd. I really hope so.

Same here, my last GSD died this same way, same symptoms. I waited, hoped, but in the end I felt selfish. I just talked to my brother and it sounded like he was having Theo PTS.

My Dr. Crusher. Had symptoms as many as you describe, she was nine year old, the conclusion after two days at the vet, was that something had bit her. So sorry to hear. Wish you well. Dan

Theo is at home, My brother is still holding on to the hope that Theo has what the second vet diagnosed, Idiopathic vestibular syndrome, which in many cases the dog recovers. When my brother held him upright
he is tilting his head, if you change his position, the eyes are moving in a rhythmic motion back and forth, but at rest they are fine. Vomiting has stopped, and he is taking water by hand, he wags his tail, and is crawling toward them.
I would give him Gatorade for electrolytes…any thoughts?

I am so glad there's some hope and improvement! If this in what it is, I believe I have read that recovery usually happens within a few days to a couple of weeks.

I'm just so glad Theo is home tonight and interacting a little bit with his family... that MUST be a good sign. Did the vets already give fluids? Can they be "overdone"?
