I'll add also that having your dog on a feeding schedule helps too. If you know when she's eating and drinking you can start to get an idea of her digestion timing is. Free feeding a dog with housbreaking issues only makes it harder for you both.
She is on a schedule for eating and exercising, but she will hold her pee and poop infinitely - in the seventeen years I have owned and worked with dogs I have never met one that will not pee and poo like this one! I have taken her on a 45 minute walk first thing in the morning after she has been crated overnight and she will refuse to poo, perhaps do a bit of pee. It really is not for believing. She has held her poop for over 18 hours, so that is a huge challenge for housebreaking.
still the challenge is the same. do not let her go in the house, no matter what.
maybe you should consider making her an outside dog for a while. have an enclosed run within view of your kitchen window for her. when you see her relieve herself in the run, then go out and lavishly praise and bring her inside. she'll learn if she wants to come inside, she has to go in the run.
it's just an idea, not something i've ever tried. but i do think you may have to get a bit creative with this one and try an unusual approach if necessary.
