if this was my dog, I would probably not take him to the vet at this point unless you think there is an underlying medical issue.
If you do some reading, you will see that flank sucking is really not dangerous as long as the dog isn't damaging himself if and when he does this.
I think many vets may go the "meds" route with something like this, and I would not want to medicate my dog for this unless it was REALLY interfering with his day to day life.

I got to thinking last night (YIKES) but my Dutch will do this to stuffed animals when we are relaxing in the house (usually pm when we settle for TV). Whether it is in a crate or not.....
He does not suck his own flank, but rather puts his whole mouth over the stuffed toy and has at it. Otherwise it is the "place ball on lap and stare at mom til she puts me in a crate" behavior. (although he is getting better at that)
The best part is he now settles nicely on his "memory foam" doggie bed without a crate........ that is progress for this dog.
I try and make sure they are put up, the weird thing is, he will choose a stuffed toy over a cream cheese filled kong......

I did hesitate bringing him into the vet, just becasue it's not harming him, and he is a wonderful, happy, healthy dog who gets a lot of excerise.
If it progresses, I'll seek vet help, or opinions from vets, but right now he will only do it liturally once a month.
I plan on giving him even more excerise, and training even more (Good Lord I already spend all day with him hahaha)
And I will also be keeping him away from PB just incase that set him off.
